Life Certificates

The Consulate provides attestation services of Life Certificates for purposes of pension drawn in India.

Applications for life certificates may be submitted in person with prior online appointment at the Consulate or may be sent directly to the Consulate through mail/courier at the following address:

It may be ensured that the applications are complete with the following:

  1. Duly filled in Life Certificate form (for blank form click here)
  2. Miscellaneous Service Application form (for blank form click here) – Kindly ensure that a WhatsApp mobile numberis prominently written on the application form.
  3. A self attested copy of valid passport.
  4. A self-addressed, prepaid Xpresspost/courier envelopemust be attached in order to dispatch the authenticated life certificates back to the applicant address.
  5. Fees: (i) Life certificate for pension purposes is provided free of any charge.

(ii) For all other purposes, application for Life Certificate (non-Pension) should be submitted with BLS International Services Ltd. It may be ensured that the applications are complete with the following:

(a) Duly filled-in and signed Life Certificate form for non pension purposes along with copy. [Click here for Form].

  • For Indian passport holders submitting in-person application: No need of notarization/apostille.
  • For Indian Passport holders sending by mail life certificate should be notarized.
  • For Canadian passport holders life certificate form should be apostilled from concerned provincial authority/Global Affairs Canada before directly submitting in India.

(b) Miscellaneous Service Application form (for form click here) – Kindly ensure that a WhatsApp mobile number is prominently written on the application form.
 (c) A self attested copy of valid passport.
 (d) A self attested copy of address proof (DL/utility bill/provincial ID, any one)

(e) Fee CAD 17.00 plus BLS Service Charges.

(f)  Status proof in Canada of the applicant.


It may please be noted that incomplete applications shall not be processed and would be sent back in the return envelope provided by the applicant.

Consulate will identify the applicant through a video call over WhatsApp before authenticating the Life Certificate. The authenticated life certificate would be sent back to the applicant in the pre-paid self-addressed envelope provided by the applicant.
